DCB0129 Clinical Safety Playbook
DCB0129 is the NHS clinical safety standard for health IT. It requires a Clinical Safety Management System, a Clinical Safety Officer, a hazard log, and a signed-off safety case before you go near a procurement conversation. I've built all of those for the first time, from scratch, as a solo GP-founder — this is the working account.
What's in the playbook
- Nine milestones from CSMS setup through to post-market surveillance — each marked as lived, in-flight, or not yet reached, so you know how much to trust each section.
- What a real hazard log looks like for a referral decision-support tool: 14 hazards, initial and residual risk scores, and the reasoning behind the risk controls chosen.
- Two anonymised production incidents from July 2026, worked end-to-end — how they were detected, graded, escalated, and closed within the CSMS framework.
- Where the CSO independence problem sits, and why it's the single biggest blocker between a solo founder and a defensible procurement submission.
Scope
This is a free preparation aid, written from my own clinical-safety practice. It isn't certification, isn't a substitute for DCB0129:2018 itself, and isn't clinical-safety advice for your specific product. Every milestone links back to the official NHS Digital source.
